Taking part is voluntary. You can skip any question or leave before submitting. Please leave out names, student IDs, and personal health information.
The MCW project team, Dr. DeBisschop, and collaborating PSW staff will use responses to evaluate and improve the campaign. They can review written comments and optional school and year selections.
This browser keeps a random response code so your first and follow-up surveys can be matched. Your answers are sent only when you select “Submit response.” For project questions, email Dr. Michael DeBisschop.
